Orchard Park Rehabilitation and Living Center is located in a residential neighborhood close to Farmington Village, on a quiet street corner. This small, homelike facility boasts a combination of short-term skilled nursing (following hospitalization), rehabilitation including speech pathology, occupational and physical therapy, long-term and end of life care, as well as unique apartments providing independent and assisted living. We also can arrange outpatient rehabilitation after a nursing stay or directly from the community.

Individuals residing in our apartments have access to a range of amenities including personal care, medication administration, meals, housekeeping, activities, transportation to shopping or doctors’ appointments and laundry services.

With this wide range of services, our patients can move from independent living to skilled nursing (following a hospitalization), rehabilitation and back to independent living, as well as long term care – Whichever level of care is appropriate for you. We accept Medicare, Medicaid (MaineCare), private insurance and private funds depending on eligible services. The nurses are nice the place is clean. The aides are awful!!!!! The patient care is awful!!!!!! My mother in-law had a major stroke so we placed her here at Orchard park because their rating was suppose to be so good. Not true she was left in a room too die is the best explanation. Every time we came visit which was almost every day. Her door was closed her light was always off in a dark room. They didn’t bath her either. The only time they combed her hair or cleaned her up was after she passed away. I would not recommend this place at all!!!!!!
My grandmother stay has been so much more then I ever expected. She came in extremely weak, I thought she would never be able to return home. The nurses were extremely compassionate and gave us hope. The therapy department gave her encouragement and worked with her daily. She is able to return home. All of the staff made my family feel as though we were going through this together. I would recommend this place to anyone who needs rehab. They genuinely care about their patients and their family members.
The gal at the front desk was awesome. Very helpful and accommodating. I felt welcomed for sure. My mother-in-law has only been in the facility for two days, so I am sure I will be providing another review regarding her stay and how she was treated after she comes home. Initially she is pretty happy with the daytime staff, and has not had the same opinion of the nighttime staff so far. Will follow up after her stay and let you know how her overall experience was. If it's like I was treated at the front desk all will be good. Thank you.
Hello everyone who had a mom or dad who recently had a stroke. My mom had her 4th stroke and was a patient at Tacoma General Hospital. My mom was discharged and ready to go to this rehab place. Once I arrived, I checked in at the reception. The reception lady was rude and with short patience. Once I walked back to where my mom's room she was sitting on her bed alone. My mom is a fall risk, so felt this place is extremely unsafe. I can advise everyone if you truly love your mom and dad don't send them to this place. This place is short staff and they don't have that many of patients. If you have family that may go here. Go by and check to see how HORRIBLE THIS PLACE IS.
My mother has been at Orchard Park twice. The facility is not new, but it is clean. She tells me she is well treated, that the staff is patient and respectful, the food is fine, and she enjoys the activities. I have found the case manager and social workers to be helpful and prompt to return calls. Laundry service is ridiculous - make sure you mark a last name clearly on all items to stand a chance of getting them back - better, plan to take it home, wash there and bring it back next visit. Mom is treated well and kept comfortable. I can handle the laundry.
My father is being cared for here after being hospitalized with a serious illness. He has received exceptional care, in all areas. He has enjoyed very healthy and nicely prepared meals. He has had both occupational and physical therapy. The rooms are modern and spacious and clean. The nurses and other caregivers are very attentive and helpful. We are thankful for the kind and compassionate care received.
Terrible and incompetent! My father-in-law is much worse now that when he entered.. They aren’t taking him to the bathroom in a regular basis, they are now keeping him in a diaper all day which he didn’t need before. They were told by my mother-in-law that he’s fully covered by the VA. Orchard Park never submitted his paperwork to the va for payment and hit my mother-in-law with a $5000 out of pocket bill. They told her they will then submit the bill but they won’t pay retroactive and she is responsible for the bill. They also told her that if he stays there they will put a lean on her home. Our family decided that we are going to get him. We’ll now they say we can’t because they just put an IV in him. We let them know we’d be coming to get him in a few days. Make sure to check with Department of Health, BBB, and Attorney General before closing this facility. I wish I would have checked even just the Google ratings before putting him in this horrible place.
Reviews on a facility like this will always be skewed towards the negative just by the nature of their business. I looked at my options for the care of my mother since she was not able to manage herself any longer at home and Orchard Park hit the top of my list in Tacoma due to their ratings for medical issues and care on Medicare.gov. They have the best ratings and that's what I wanted for my mom - the best chance to survive and thrive. Now, my mom had to adjust to life here. She wanted to go home every day for the first 30-40 days, but when things got rough, that's when I'm REALLY glad that I chose Orchard Park. They contacted me when it was needed. They listened to my concerns about my mother's care, and they gave me the best options and allowed me to process what was happening with my mother as best as anyone could. Look, this is a very hard service to provide. There is A LOT of need, and VERY LITTLE thankfulness. My mother lived 3 months in the care of Orchard Park. During that time, I was able to go to work, have a life, and still keep up on my mother and her care as much as I chose to. My mother was dying. I did NOT want her to have a terrible experience towards this eventual end. The staff at Orchard Park helped me to help her go with dignity, peace, and comfort. They called me when I was supposed to visit her on Tuesday and said that she might not make it to Tuesday. I visited on Saturday with my wife and we had SUCH A GREAT last visit. I will always remember how my mother looked at my wife and I with such satisfaction - knowing that I would be loved long after she was gone. Those memories are priceless. Orchard Park then called me again on Monday night to let me know that my mother was not waking up. We then headed over again to say goodbye. She was gone the next morning. I sincerely thank all of the staff at Orchard Park for assisting my mother through her last days. It's not an easy task. They did it with respect and responsibility. I could not have asked for anything more from them.
I really love the staff! Very nice and very helpful for my mother. She went there for physical therapy and is staying 3 weeks total. The therapists are great! They know the goal and I believe that she’s going to be ready to come home when the time gets here. Thank you, Sincerely, Pamela Bateman
The care team here is very patient and attentive. The facility is quiet and well-managed, even with as many patients that they care for. They are able to get my grandma to take her medications daily, as well get her to eat even just a little bit of food--two things which were neigh impossible when she was at home. Dementia can be a dirty thing, but the care staff here really know what they're doing.
