Personal Care Medical Associates is a team of physicians, nurse practitioners, a PharmD, nurses and support staff whose complete focus is on providing the best care to patients in the facilities that we serve. Our quality program is second to none. We also perform chromic care management and advance care planning.
We are a strong partner to health plans, hospital systems, ACO's and physicians. We strive to treat people in place, avoiding unnecessary hospitalizations, reduce facility length of stays and decrease costly diagnostic tests and treatments that will not positively impact patient care. We work with out partners to ensure proper and safe transition for our patients. We also emphasize palliative services and are always keeping patient goals and values at the forefront or our treatment plans.
