Reach is an established home health agency. Our organization was developed in response to patients who needed assistance with health care and personal needs and wished to remain in an independent living environment. We provide a cost-effective alternative to skilled care by utilizing highly trained caregivers as an integral part of our healthcare delivery system.
Starting with a thorough evaluation by a Registered Nurse (RN) Case Manager, we determine the patient’s health status and medical/personal care needs. The RN will determine if the patient’s needs can be met by an unlicensed caregiver based on the stability of the patient’s status, the nature of the tasks, and the training of the caregiver. The RN will develop a plan of care for the patient including the frequency of supervision required.
Reach Healthcare Services provides a large range of services to an extremely diverse patient group. Genetics, disease, accidents, and the normal aging process affect each person differently, yet the one thing that all of our clients have in common is a desire to live as independently as possible, in their own home. They also don’t want to feel like a burden to their loved ones.

Medicare Home Health Programs
Wound Care Program
Our Wound Care Program includes weekly in-home assessments with reporting and light therapy as an adjunct treatment to physician’s wound care protocols. All treatments are administered by Level II trained wound vac nurses or therapists.
Diabetic Education Program
We offer a Diabetic Education Program for both non-insulin dependent and insulin dependent diabetics. Our Non-insulin Dependent Program is a three-week intensive training with diet plans, exercise, glucometer testing and covers pancreatic function and foot care. The Insulin Dependent Program is six weeks of intensive training including all topics covered in our Non-Insulin Dependent Program in addition to insulin types, preparation and injection. Both programs also provide coordination of glucometer and testing supplies.
Geriatric Rehabilitation Program
We offer Geriatric Rehabilitation Programs for knee, hip or shoulder rehabilitation following a joint replacement, balance improvement to reduce falls and neurological rehabilitation. Our Geriatric Rehabilitation Programs improve functional independence and can also treat individuals with multiple sclerosis or who experienced a stroke.
Congestive Heart Failure Program
The Congestive Heart Failure Program covers cardiac function, symptom management, essentials of medication, diet pitfalls and life enhancement through exercise. We also provide cardiac rehab through physical therapy, if appropriate.
Private Duty Nursing and Caregiver Services
At Reach, we offer a cost-effective program for individuals who wish to stay in an independent living environment, but require medical care from a trained professional. Starting with an evaluation by an RN, we determine medical needs and discuss if these needs can be met by an unlicensed, trained caregiver based on patient stability. This service is most commonly used in cases of spinal cord injuries, traumatic brain injury, congenital disorders and motor vehicle accidents.
