Sports Registration Documents
To participate in official practices and competitions, students must have clearance from the Skyview Athletic Department. Students will need to submit the following documents in person to the Athletic Office. All forms are available in the Athletic Office
1.Proof of medical insurance.
2.A current physical examination form stamped and signed by a doctor licensed in the State of Colorado. Physicals are valid for one calendar year.
3.A signed student/parent athletic code.
4.A permission form signed by parent/guardian.
5.Payment of the athletic fee:
◦$40 per sport for high school sports
◦$20 per sport for middle school sports
All documents must be complete and on file in the Athletic Office before a student may participate.
Academic Eligibility
The Colorado High School Activities Association (CHSAA) requires students to meet personal behavior and academic performance standards for participation in interscholastic activities at the high school level. General eligibility guidelines are as follows:
Weekly Grade Checks
Each high school will monitor a student athlete's grade and report information to the Athletic Director each week. A student must be passing (D or higher) 5 for-graduation-credit classes each week in schools using the quarter and/or semester system. In schools using trimesters, a student must be passing 4 for-graduation-credit classes each week.
If a student does not meet the weekly standard for eligibility at his or her school, the student is prohibited from participating in competition for a seven day period.
Semester Eligibility
In addition to weekly grade checks, a student must have passed 2.5 Carnegie unites of credit each semester or trimester equivalent.
If a student does not meet the semester/trimester standard, the student is prohibited from participating in competition for approximately the first six weeks of the next semester.
NCAA Eligibility Center
Any junior or senior wishing to compete in a Division I or II NCAA athletic program must be registered with the NCAA Eligibility Center and certified as eligible. Student-athletes should inquire at their individual school about how to get registered. Their website is https://web1.ncaa.org/eligibilitycenter/common for additional information.
Varsity Letters
Athletic letters are awarded for participating at the Varsity level in a high school sport. The head coach of the sport you are participating in will let you know his/her requirements for earning a Varsity letter at the beginning of the season.
Physical Education Credit
Physical Education Credit may be eared if a student-athlete completes the season and satisfied the attendance and participation requirements set by his/her coach.

Fair play, courtesy, generosity, self control, and friendly feelings for the opposing team or school shall not be sacrificed in the desire to win. The Mapleton School District will take every possible measure to assure courteous, friendly, and fair treatment to visiting players, officials, fans, and game officials.
As a Mapleton student, you have the opportunity to compete in a variety of junior high and high school sports as a proud Skyview Wolverine. Competing as a Skyview Wolverine takes time, energy and focus, but the rewards are great. Mapleton Public Schools invites all eligible students to participate in their favorite sports and to tryout for a new sport in another season.

Middle School Sports
Middle School sports are primarily offered to Mapleton students in grades 7 and 8, however 6th graders may participate in Wrestling, Soccer and Track and Field. Contact the Athletic Office for specific start dates, practice times and locations.
