Share Our Stories© (S.O.S.) has been designed and developed as an easy and quick online reporting system for anyone who has experienced or observed a medical incident that has caused a harm or could have caused harm to the patient or others.
Stories gathered from S.O.S. will be compiled into the Patient Safety Registry to study the number and variety of personal observations, satisfaction, and lessons learned reported by healthcare providers, patients, and their families. We encourage everyone to use S.O.S. to the fullest capacity of sharing his or her stories of the most recent medical incident. While we tend to recall the most harmful or potentially harmful experiences, S.O.S. is also designed to capture those personal stories that are positive and preventive when someone performs a safe behavior and action.
