FAILURE TO BRING ALL OF THE FOLLOWING WILL RESULT IN THE INABILITY TO FULFILL THE STATE OF MICHIGAN REQUIREMENTS FOR YOUR CERTIFICATION.
-Picture ID
-Medical Records (copies that we can keep for your file) documenting chronic health condition(s)

-Payment at time of service:
-new or 2-year renewal office fee--$125 debit/credit card or $115 cash discount
-annual follow up visit (required)--$75 debit/credit card or $70 cash discount
Compassionate office fee for SSD or SSI recipients $100
Compassionate office fee for veterans $100
-Also must bring a check or money order for the application fee written to "The State of Michigan" in the amount of $60
OR
-$60 cash and we will write the check for the application fee
-$60 fee to The State of Michigan is required every 2 years
