Description

Dr. De Carvalho is a Medical Doctor trained in Psychiatry. He brings a wealth of knowledge on psychiatry and plans on utilizing a holistic approach to mental health treatment. Some of his main interests are Schizophrenia, Bipolar Disorder, Depression, Anxiety, Addiction, PTSD and ADHD (adolescents from the age of 13 and adults). He has strong training in substance related disorders, including substance-induced dependence and substance induced mental disorders.

Mission

In July of 2012, Orange Park welcomed a new psychiatrist to its community. Dr. De Carvalho’s outpatient practice offers patients a calm, comfortable and relaxed environment to explore options for the evaluation and treatment of behavioral and emotional concerns. Whether it is “something I’m going through” or a longer term pattern of behavior which is adversely affecting family relationships, educational or financial success, or the development of appropriate social skills, it is always helpful to seek the professional opinion of a psychiatrist.

The staff isn't the least bit concerned or compassionate considering the patients are coming for help with their mental health. They are snotty and cold on the phone.
When you first visit this practice it seems elite and professional. I initially saw DeCarvalo. I was kind of forced to go to this doctor when my original doctor died suddenly. They took my insurance but always overcharged my coinsurance. No matter how many times I told the manager “Ginger” that she was billing me incorrectly and that I worked at Florida Blue so I knew my Copay etc, the more she simply ignored me. I brought my frustration to Dr. Decarvalo, and he advised me that he would take care of it. Three more months of over billing me, not to mention they were now billing two procedure codes as if you had an extended visit!! They bill a regular office visit procedure code: 99214 and a psych visit 90808. It was then that I realized these people are crooks! I saw this doctor for all of three minutes while he looked at his phone or interrupted the visit every time I saw him. I remember telling “Ginger” that my deductible was met it was March and she asked from the window “how did you meet it so fast, did you have surgery or something?” I was horrified. All she had to do was verify my insurance but insisted she only calls “New Directions” not my actual health plan who could have verified everything I was telling her without the need for asking me questions that were none of her business. Fast forward another six months and I was seeing another doctor “Dr Brian Beardsley” who is the best! Kind, compassionate, CARES ABOUT YOUR WELL BEING! Also within the same practice because DeCarvalo was not available. Ginger said they don’t honor FMLA yet Dr Brian Beardsley said “yes” he did honor FMLA, sent the paperwork in and I was approved! When Ginger the office manager, told me they didn’t do FMLA paperwork!!. After bringing this to Ginger’s attention, she ACCUSED ME OF “DOCTOR SHOPPING!!” I can’t believe this woman even holds this title when she doesn’t know the definition of doctor shopping, coinsurance, or anything to do with insurance!!Real doctors care about their patients. This one only wants to write prescriptions and say goodbye. My daughter called numerous times, nobody calls you back. This causes you to call several more times and the answer you get is “did you leave a message?” My daughter replied “yes but I need an answer” and the person at the front desk said “you need to learn to follow instructions, that’s why you haven’t received a callback, because you keep calling, and your holding up my line of patients!” SERIOUSLY??The the over billing continues at both locations. I was billed up front in January for the FULL AMOUNT OF THE OFFICE VISIT, when I tried to explain that since they were in network, they were supposed to charge me the “allowed amount.” The front desk couldn’t comprehend what I was saying so I was forced to pay the whole thing. The straw that broke the camels back was when I was was admitted to the hospital and they contacted Decarvalo and he refused to see me advising other doctors at the hospital that “I was faking!!!” I’m sorry but WTH??? So I came off the meds on my own and notified insurance company of fraudulent billing. They are monitoring every bill that comes from this practice and eventually he won’t be able to bill insurance at all!! They DO NOT GENUINELY CARE ABOUT THE PATIENT!! You got a problem? They dismiss you from the practice. Totally UNETHICAL, and the office manager knows nothing about coinsurance! You wait an hour for Decarvalo, oh well! But you cancel your appointment and they will charge you $90.00 in a New York second! Don’t you dare question it, or you will be scolded like a child! Office staff have ZERO customer service skills what-so-ever! They do not care about you as a human being!! You are just a dollar sign! Oh, and WHO prescribes Gabapentin for anxiety?? My hair started falling out! This wasn’t healthy & was not good for my well being! I didn’t want to leave one star but had to! RUN, RUN, RUN from this place. Dr. Brian Beardsley is the best, Decarvalo turns a blind eye!! THIS PLACE SHOULD HAVE BEEN CLOSED DOWN A LONG TIME AGO!
Thanks to Dr. D and Louise I'm back on track! If it weren't for Louise I'd still be a hot mess of stress! The office staff is great - I've never had a problem with them. They are always friendly and smiling - but then so am I when I approach them. I've seen these poor ladies deal with some real 'characters' - which I guess is expected at a Mental Health and Psych office - and they have always handled it professionally. Kayla keeps the Southpoint office on point! Definitely recommend!!
I've been seeing Dr DeCarvalho now for about 8 months, and while I think he's a pretty good psychiatrist, the office practices here are HORRIBLE. There is no way to actually get in touch with him if you need to between visits because the phones are never answered and any messages you leave are never returned. The online patient portal is just as useless. Appointments are always at least 45 minutes late, so be prepared to sit in the tiny waiting area for a while.
I don’t typically leave reviews because I generally don’t feel the need to despite many negative and/or positive experiences I’ve had regardless of the product or service—however, I can’t let this one go simply because mental health is very clearly NOT taken seriously whatsoever at this place. I don’t have insurance currently, so this was the only local psychiatrist I could barely afford out of pocket. I don’t necessarily have an issue with the doctors and therapists themselves, because I understand working in the medical field is extremely time consuming, and I don’t expect to have constant communication at the drop of a hat nor do I hold any unrealistic expectations of the practitioners over their heads. With that being said, I can honestly, without a doubt or an ounce of guilt, share my displeasure with my course of treatment at this facility both locations. I will call and leave voicemails to make appointments, request medication refills, and sometimes ask general questions, as any established patient would—but regardless of how many times I call, how many voicemails I leave, how many attempts I make to reach someone at either office I am 9 times out of 10 ignored. I almost never get a call back, my medication refills are always late and not only that, I have also been called by a blocked number and quite literally been cussed out by one of the female office staff whose name was not shared with me prior to my being promptly hung up on. I suffer from SEVERAL severe diagnoses. I am prescribed large doses of medications that cannot be stopped cold turkey without risking fatal withdrawal and side effects. It’s been made very clear to me by numerous doctors that the meds I am prescribed would require hospitalization and closely observed weening in order for me to, without risking potentially fatal outcomes, be taken off of them. Almost every month if not every other month I will run out of my medication, after requesting a refill a week ahead or more at a time to ensure I do not miss a dose... and yet I somehow still end up going a week to sometimes 2 weeks without despite my numerous attempts in reaching out to communicate refill requests. I have been told, when I’ve been able to get a hold of someone in the office, “you’ll get your refill when you get your refill, and calling us multiple times will not refill your meds any faster.” I know for a fact that they screen my calls, because if I call from the phone number they have on file for me, I’m always sent straight to voicemail—but if I use another phone with a number they don’t recognize, they pick up. The girls at the front desk will smile and act sweet and professional in person, but God forbid you get a hold of one of them on the phone... then it’s a totally different story. The inexplicable unprofessional and “Mean Girls” level attitudes I’ve been subject to is absolutely unbelievable. Doctor De Carvalho is definitely a great doctor, but he’s also fast to prescribe medication as a quick fix before shooing you out of his office after a maximum of a 5 minute “session” with him. He’s never been rude to me, but he doesn’t seem to take an interest in me as a patient—his bedside manner definitely needs some work, especially when I have personally expressed my concerns to him regarding his staff and felt by far disregarded in response. I honestly don’t know how this so-called “Center for a Healthy Mind and Well-being” has been able to remain in business with how his staff treats the patients. Each and every one of the negative reviews on here are spoken truths to no exaggeration. I can almost promise you that the most recent reviews that specifically named the girls who work in these offices, shouting praise over their “professionalism” and “compassion” are more than likely friends or family trying to drown out the bad reviews—I should know, because I am a professional woman myself and I can tell you firsthand that it’s an overused tactic to redeem their rapport. Somebody needs to take action, because the way this place is run will ultimately result in a lawsuit.
This practice habitually cancels and reschedules visits, electronically by email and text, especially for Dr. D, who also habitually is late, sometimes up to 2 hours, but at least 1/2 hour, in my experience. Since I became a patient a year ago, 2/3 practitioners have left the practice. Apparently they recently closed the OP office except for Monday's, and have no staff except for Dr. D. himself. Phones purportedly are forwarded to the SS office, but as a patient I received no notification of the changes. When my insurance company called 4 times to confirm they were paying my claims, they couldn't even get through to anyone. I personally gave the office the relevant insurance reference number and asked them to call my insurance company to resolve billing issues so that the office would schedule my most recent appointment. The office refused to contact the insurance AND to reschedule me. When I explained that I am a patient in need of medical care specifically new prescriptions, the SS receptionist yelled at me, interrupted me repeatedly, and refused to answer any of my questions or even listen to my billing request. I left a lengthy and detailed voice message for her boss, who also happens to be the office manager and person with whom I was told to speak to resolve the billing issue.She never returned my call. I guess they don't care if patients run out of medications, which can be life threatening. I wonder if Dr. D. Even knows that his office is more concerned with billing and insurance protocol, even when they're in the wrong, more than his patients themselves. less »
This is a doctor who ends up with you in two minutes after paying $ 100 the two times I went and prescribed a medicine that my insurance wasn’t cover I told him to prescribe something that my insurance covered, but he did the same thing again I waste my money they never answer the phone at the office, I really do not recommend this place , nothing professional, not like or interested in the patient only interested in money when I start reading the comments about him every is true . I have been seeing Melinda B. for about 1.6 years and she is caring and compassionate. She cares for her patients. They also have an amazing front office team, ran by 2 lovely professional women. They go out of their way for their clients. They are more than pretty faces answering phones. They are the backbone of that office that keep the business running smoothly, like a well oiled machine. I hope the doctors realize what an asset they are to the practice as a whole. I always recommend this practice to my friends. Thank you all for being the dream team. I enjoy seeing them each month.
I’ve been waiting 2 weeks for a prior authorization and I have made several trips to the office they never answer the phoneto remind to do their job which is only 1 phone call to BCBS she says they’re going to do it but never does. They are so incompetent and unprofessional. Avoid extra stress and stay away from this place!
Main problem is the office staff in both locations. They have a terrible attitude toward patients. I’ve put up with the front staff for years, because I liked the nurse practitioner who saw me...but really, it’s not worth the aggravation. Instead of feeling a healthy mind, you just get stressed out. This week, I had a couple of issues and I’ve been waiting for a call back for a long time. I went in person and said that my call wasn’t returned. Instead of taking responsibility for the inaction, the lady at the front desk was very defensive and in an overtly rude manner told me that they ALWAYS return calls. OBVIOUSLY, as you can read in other reviews, they either not answer the phone I also called several times and/or they DON’T ALWAYS return the calls. The problem I had was not fixed and there was no will to help. That was my last time there.
