Umpqua Community Health Center (UCHC) provides primary medical, dental and behavioral health services at six locations throughout Douglas County.
UCHC is a preferred provider for most insurance companies and we welcome Medicare and Oregon Health Plan Medicaid patients. But don't be concerned if you are uninsured or have a high deductible. Our services are billed on a sliding scale basis based on income and family size. Many patients qualify for discounts and/or payment arrangements. No patient is turned away for the inability to pay.
UCHC is a Federally-Qualified Health Center and a recognized Patient-Centered Primary Care Home, providing not only primary health care, but a variety of support programs and resources to help you achieve your health goals.

Hours: Tuesdays from 1:00 – 4:00 pm
Immunization Services & Hours:
Child and adult immunizations provided.
Children under 15 receiving immunizations must be accompanied by a parent or legal guardian.
