Glendale Uptown Home Rehabilitation and Skilled Nursing Center is nestled in a quiet, tree lined neighborhood in Northeast Philadelphia. Glendale Uptown Home is dedicated to providing compassionate nursing care, interesting and unique activities that stimulate the mind to generate an overall sense of well-being, and an exceptional rehabilitation team to help get you or your loved one back on your feet after an accident, or surgery. Our staff consists of compassionate, knowledgeable physicians, registered nurses, licensed practical nurses, and certified nursing assistants, physical therapists, occupational therapists, and speech therapists.

I went off. My brother bed was wet and so was he. You could tell it had been that way for awhile the stain was brown. Could have been days. We went to get someone to change bed. When the sheet was removed the padding under was all over the floor. He had no cover and some one had covered him with a pillow. He could not do it himself. His feet was cracked, dry and bleeding. He had a bed sore and sore spot on his head. His lips were cracked. We asked for lotion and my sister put lotion and lip balm on him. The worker I spoke to had only been there a short time. The two workers on floor looked exhausted. If you can’t go there daily to check on your love ones I would not recommend you put him here. I live out of state and could not come daily. He will be leaving in a week. I am getting him out. The next rehab will be close enough that family can pop in unannounced. I needed to vent.
This place has been such a wonderful experience for my father and my family. My father was living on his own until he broke his hip in June. I was terrified at the thought of having him cared for by a facility and it’s staff and not myself and my family. Now that my father has been at Roosevelt Rehabilitation since July, I can say with 100% confidence, I made the best choice ever. I would like to thank Dr. Nadiya L for helping me to enroll my father. She was very understanding, compassionate and professional from day one.I cannot thank you enough Dr. Nadiya and The Roosevelt Rehabilitation community!
My grandfather received exceptional care while at this facility. The staff, in all capacities, were always pleasant as well as helpful whenever called upon. My grandfather said they were always that way even when family wasn’t around. He also said that he would miss them when he returned home… I think that says it all! Certainly deserving of 5 STARS!
I must say I did leave a one star review previously; however, my attitude has changed immensely about Roosevelt. Marie, the aide, was the kindest and nicest person I have ever encountered. She took a excellent care of my mother. The nurses and physical therapists worked with my mom were beyond professional. The social workers helped us so much. Thank you to the staff of Roosevelt!
My mother 99 years old after hospital great services. Russian speaking concierge is wonderful Gary been by myside since day one. A lot of progress with therapy I couldn’t be more thankful! This place is recommended with guaranteed progress ! Amazing job thank you Roosevelt
My father was admitted to this facility for rehabilitation after a debilitating illness. During his stay our family visited him quite frequently at various times of the day and night. We always found it to be clean and well staffed with both nurses and CNA’s, but moreover, we found our father to be well cared for, and with only pleasant things to say about them! I believe that everyone is entitled to an “off day”, but I never came across this with any of the staff while my father was there. If I could give more than a five star rating I certainly would!…
This place is horrible. My mother in law was here for about 3 years. When calling that place no one likes to answer. And when they do they either hang up on u or are rude. Everytime we would go see my mother in law she looked like she wasn't bathed. She barely ate. She was super skinny. She just passed away on Dec. 3rd. We told the nursing home the same day. We just went there today to retrieve her belongings. First we was told they couldn't find her stuff. Caused they took it out of her room a week ago. Why the hell would u take her stuff out of her room while she was in the hospital still alive for. If she would have still been alive they would have put her back there. Who the hell gives you the right to move her things. And then tell us u can't find them. Then they found her stuff thrown in the basement with other people's things thrown on top of hers. Then we went home and went threw her stuff to find things missing. Tried calling them an no one will answer. And she was severely under weight. I wouldn't recommend this hell whole to anyone.
Don't send your loved ones to this facility my aunt has been in here for 3 weeks not only does she sit in soiled and wet diapers. The facility will not give you a phone number to the patient's room she has not had ATV in her room the only way to connect with her is to call the other patient in their room to get her to use their phone I have addressed this with administration who assured me that the problems were being taken care of then we had a meeting with department heads last week and 3 of the department heads were out sick They promise to give my aunt PT every day and she still is not getting it I cannot reach anyone by phone by email or any available resources. I live in another state and can only visit once a week and when I do visit the stories she tells me are just terrible how they treat the elderly. I'm hoping that your facility reaches out to me after reading this post this is my only way hopefully to get some action for this place or else my next action will be reaching out to the state.
My husband was here and this place legally should be closed, it was horrible. He had cold food daily, no interaction with any type of staff. Complained to Sr staff , nothing changed. They bill for 40 minutes of therapy 5 days a week he only received maybe 10 minutes once a week. Isn’t that insurance fraud. And the food, small extremely cold portions, u our food can sit in your room or on the cart for hours, staff claiming no way to heat it up, he said the food was almost unfit to eat. Other reviews stated family members lost so much weight. How do they get away with such treatment.Took days to get pain pills or any medication. It was so bad he signed himself out taking an Uber to a better facility, They get - stars, negative stars
Hear Ye. Hear Ye. WARNING: DON'T, DO NOT think about placing your family member here. My family member almost died at Roosevelt Rehabilitation and Healthcare Center. This is a horrible place where patients are victims of abuse and neglect. Patients at Roosevelt Rehabilitation and Healthcare Center are dying from neglect, they are malnourished and losing a lot of weight from not being fed or being fed garbage. Medicaid and Medicare should stop making payments to this dump and the Department of Health should close this disgusting place down. The examples of horrible, sub-standard care at this place are more than sand on the beach. So, I'll just list a few. 1. You'll never get to speak with your family member because patients do not have phones in the room and NO ONE answers the phone on the unit. No matter what time you call, you will just receive a recording that the number is not accepting calls. 2. Patients are not being properly fed, they are malnourished. This place is maximizing its profits by serving cheap, low-quality food and frequently, meals do not include meats or any source of protein. The attached picture is an example of an entire meal - runny macaroni and cheese with a glob of tomato paste; Yes. Tomato paste, not sauce. As a result, patients are frequently hospitalized because their bodies are too weak to prevent or fight infections. 3. Patients are left to sit in their soil diapers, which results in UTIs and other life threatening infections. 4. Prompt medical treatment is not given which results in patients developing sepsis. 5. The facility has a rat/rodent infestation. There is a saying that the fish stinks from the head. Roosevelt Rehabilitation and Healthcare Center is a prime example. The Administrators and Managers do not care about providing quality care to the patients in their care. You will not have anyone to whom to voice your concerns. The Social Work and Nursing department managers often do not answer the phones and they do not return calls. The Administrator Jonathan and Henry NEVER, EVER answers the phone and does not call back when messages are left for him. If you ignore these warnings and place your family member here, it is a decision you WILL regret for the rest of your life.
