This will be the last time I even attempt to go to this Dr. The lady who took my information at the window didn't know what she was doing wanted me to out my credit card information on a sheet of paper so they can keep it in my folder that's not very secure but if I don't do that I have to pay in full with cash and provide my peia insurance I wish they would have told me this in the first place no way am I giving them my credit card information to hold on file and folder I would not have missed work to fool with this. Here's the kicker I'm hoping it's not melanoma on my foot but just imagine how much money for employer is going to lose if it is. Dissatisfied customer
