I’ve had services from both Jaymee and Daphne! They’re both fabulous and took such amazing care of me. Recently I had a hydrafacial with Jaymee and it was so relaxing! My skin looked amazing immediately after! I couldn’t be happier.

I had a Frax treatment done a few months ago and I am so pleased with the results! The staff was super friendly and everything went smoothly. I will definitely be back for other treatments.

Jaymee did an amazing job with my hydrafacial and dermplane! My skin feels amazing and soft!! Daphne is also so amazing!! They both know how to take good care of your skin!

Dr. Powers is amazing! She is so very knowledgeable and genuinely cares about her clients, making sure they are happy with their results!

Form Spa has wonderful staff and services. They take the time to explain each step that they are taking and make sure that you are comfortable. It is a beautiful space and the professional services they offer are outstanding. I can’t wait to return and be beautified by Heather and Daphne again!

Jaymee was great, she was using IPL laser on sun damaged skin for me, she kept me informed along the way so I was comfortable and I loved the results. I will be asking for Jayme on my next treatment .

Daphne is AMAZING! I drive from Herriman to Cottonwood Heights to just to see her. Daphne is so sweet, kind, caring and knowledgeable that it's worth the drive.
