Heartland Health Center Inc. (HHC) focuses on health services for people who are under-insured, un-insured and those with barriers that need health care.
HHC is located in Grand Island, NE and offers Central Nebraska full-time access to care for both acute and chronic illnesses. HHC actively seeks to provide a medical home for individuals who have been unable to establish such care due to financial hardship. By providing a medical home to those who have had limited or nonexistent health care HHC is working to increase patient participation in their health care.
The mission of Heartland Health Center is to serve low-income and un-insured individuals with access to affordable primary care, through a coordinated community-wide effort.
