Midtown Health Center is committed to providing the highest quality of health care possible to the residents of Norfolk, Madison, and the surrounding area. We are committed to serving the insured, uninsured, and under insured. A sliding fee scale is available to make treatment affordable to those without insurance or on a limited income. Fees are calculated based on household size and documents annual income level. All patients, even those with insurance are welcome to apply for a sliding fee to help defer out of pocket expenses.
Proper documentation must be provided to our office to determine and slide qualification. Items that must be presented include:
** Verification of name/address (i.e. bank statement, utility bill, any thing that has been processed through the mail that shows your name/address)
** Copy of most current tax return, complete with w-2's
** Copies of 2 most current paystubs for all wage-earners in the home
** Documentation of any monthly income from unemployment, social security, disability, veterans or survivor benefits
** Documentation of child support recieved or paid out monthly
**List of any medications you are currently taking
INTERPRETERS ARE AVAILABLE AT ALL CLINIC LOCATIONS.
