➢ Skilled Nursing Services, which includes overall management and evaluation of care plan; observation and assessment of the patient’s changing condition; levin tube and gastrostomy feedings; ongoing assessment of rehabilitation needs and potential; therapeutic exercises or activities; gait evaluation and training; and other skilled nursing services
➢ Physical Therapy Services, which includes the treatment or management of physical disability, movement, malfunction, or pain by exercise, massage, aquatic therapy, and other therapeutic treatments, without the use of medicine or surgery.
➢ Speech Therapy Services, which includes assisting children and adults with deficits in communication, gross motor, fine motor, sensory, or academic skills as well as providing self-help skills.

➢ Hospice care offers comfort and support to those nearing the end of life. Hospice is a care philosophy focused on reducing suffering rather than curing a condition. Hospice addresses physical, spiritual, social and emotional needs of dying individuals and loved ones. Hospice care can include pain medication, therapy or counseling.
➢ Personal care, which includes assistance with activities of daily living (ADL), such as bathing, dressing, toileting, grooming, mobility assistance, incontinence care, feeding and medication reminding.
