Amazing at home primary care! We are so grateful to Dr. Roseline Saint-Forte for her superior at home care of my 90 year old mother. My mother was comfortable and felt safe and cared for during Roseline’s visit. She is kind hearted, patient, and knowledgeable. Primary Care at Home has been taking care of me for more than 18 months. I am very satisfied with their support and service. My Nurse Practitioner, Roseline Saint-Fort is very kind-hearted, loving, caring and understanding to my needs. If there were any tests, refills on prescriptions or anything else that needed to be ordered, she was right on top of it. And she followed through to complete the task to the end. Nurse practitioner Chris and nurse Colette are absolutely wonderful! They have been caring for our grandmother Alice For about 3 1/2 years now and they have helped her significantly! They truly care about her well-being in every sense of the word. We don’t know what we would do without them!
I highly recommend their services, they take great care of my Sweet Mother who's bedridden at home. It's a big relief for her and myself, not to take her out because its a very tiring, hole day ordeal and a lot of work just to get my mother to an appointment, it's very draining for both of us. Plus they always respond pretty quickly. They are efficient and professional. I can relax some, knowing they are there when needed.. We Love Chris Crutzher, ARNP, Collette, RN and the rest of the Staff. They are an awesome, caring team and wouldn't change them for the world. Thank you Primary Care at Home for making this happen.
The home health care taker Roseline is currently taking care of my grandmother and she is doing a spectacular job with her! She’s so patient and gets all the different kind of therapies and services my grandma needs squared away. So compassionate and also punctual and genuine with how she cares. I would recommend her to any of my family friends or anyone in to that matter in need of a trusted home health aide.
I am extremely grateful for Primary Care at Home. My father suffered a very severe stroke 5 years ago and has had a miraculous recovery. Chris and Colette have been part of my father’s care for several years now and have helped me navigate through so many challenges with care. They take the time to listen to concerns, provide resources and put effective plans in place by partnering with both the patients and their families. They have played a huge role in my father’s recovery and I’m forever thankful. As an RN, daughter and extremely dedicated caregiver, I would highly recommend Primary Care at Home to families seeking care for their loved ones. I ended up finding Primary Care At Home after my mom got pneumonia and needed some care at home. Sally Hawkins, nurse practitioner, has provided extremely comprehensive medical care and her scope of knowledge is very broad based. Sally’s patience, understanding and kindness with my mom is greatly appreciated! Sally’s care is above and beyond what I could have ever wished for and she is a true blessing to our family! My mother loves Sally and we hope to have her providing care for the span of my moms life! I can’t imagine a world without Sally in it because she lifts my burden tremendously by coming to our house and catching little things that my mom has going on such as a Urinary Tract Infection.. I said mom is dizzy, nauseous and is having a little blood in the urine.. Sally suggested trying Azos cranberry tablets for UTI.. it helped! Sally is incredible!
Primary Care at Home - When I request to speak to someone above my Practical Nurse with a complaint they will not let you speak to anyone higher then them! They "ALL" don't have a phone number for anyone higher .... like they want to hide from the boss .... who is the boss? Rafael Gonzalez? who knows they all hide .... and who answers these posts? An employee under the boss? Can't even go to the office .... it's a UPS mailbox! 3/26/2022 My wife is now in the hospital, no one from PCAH even cares to follow up ...... with the hospital or me! She went to ICU Unit, has been in for 9 days and counting ...... 4/05/2022 Wife died at home - non-covid .... never heard from anyone from PCAH after she returned, Rosaline never even tried to contact .... I grieve alone ....
Caring, compassionate and convenient. This practice has a great office staff and awesome nurse practitioners and medical assistants in the field. Vera Ahmad and Amber Ridenour are the best, always on top of calls and requests providing anything needed without a delay. Couldn’t recommend a better at home physician service!
My wife and I have been under the care of Chris Crutcher ARNP and Collette for over a year, and they have been a blessing for us. Chris has been very good at explaining what is going on with our health care. His concern and caring for us has been outstanding. His bedside manners has always been with great care and concern.
