1) Facility lost my mothers lower denture. facility admin suggested that whatever Medicaid did not pick up they would reimburse, Mind you the facility was negligent and admitted so.
2) My mother was physically assaulted by a male patient.. Patient struck my mother in her chest
3) All of my mothers clothing was missing, she left the facility with other patients clothing
4) My mothers upper denture was never cleaned.. She was there for 6 months and I always wondered why her breath was so foul. When she returned home the nurses aide removed the denture and we needed to open every window in the house. There was a orange yellow paste caked under the denture.. Obviously they never removed the denture for cleaning.. This could have been life threatening.
5) A friend of my mother has a loved struggling with dementia was able to get past the security guard and wonder the streets in 40 degree weather with nothing more than a T-shirt and pants. He was found at his residence and taken back to the facilty
6) My mother always smelled like urine. I complained but nothing was ever done
7) My mother broke her hip and was supposed to get therapy to walk again. She is now wheel chair bound for life. Her primary care doctor stated that she should be able to walk with the aid of a walker

This place does not even deserve one star. My words cannot explain how bad this place is. They don’t pay attention to the patients at all. The staff are heartless liars. They sit there and laugh all day at their own personal jokes between each other and look at you like you have 10 heads if you ask them a question. Don’t let your family stay here unless you want them to die there. I drove there twice everyday just to give my grandfather a glass of water. A patient can be bleeding out and they won’t help if they are on a “shift change”. Every time I came he was bleeding from somewhere new and it was never treated. If I’d ask for a bandaid they’d come an hour later and say their supervisor has to check it. I have dozens of pictures of the most herendous conditions that id love to share but they might not be appropriate and are difficult to look at.

CNR is a fabulous nursing home! As we service the vending machines here I witness how each and every resident is being taken care of with all their needs. The place is always spotless clean and the staff members are so nice and friendly

relative was there for a short time a while ago. The facility was very clean, bright, anThere were a lot of activities and very relaxing. My only complaint is that some of the staff and administrators lacked compassion and patience regarding those with multiple diagnosis. Meaning, if you have a loved one who is co-hearent and some what independent and just need some assistance or physical therapy this may be an excellent facility.for them to help them get better.
But if your loved one has dual diagnosis and need more assistance for daily activities I do not think they will get the proper help they need because the staff did not seem to have patience for serious ill people or people with various forms of illness.
Pros:
Clean, organized, and smell pleasant (unlike other nursing homes)
Residents seem happy and thriving, getting better.
Food looked good and they have a variety or meals,
Close to public transportation and public street parking. I think they have a parking lot but not sure if it's for visitors or staff only.
Cons:
Majority of the staff and administrators seemed to have little patience in dealing with residents who are very ill, or has multiple diagnosis. they seem not to be trained in that area and lack genuine patience and tolerance.
Rooms are small, outdated and has to be shared.
All Beds are extremely low to the ground, but I understand some residents are fall risk and it's better to be safe than sorry.
They are poorly trained to deal with dual diagnosis or sever problems. But I think they are great if the resident is somewhat independent, co hereunto, and able to do things on their own and just need a bit of assistance or structure.

Mom was there three yr ago awesome place 2018 different owner limited menu ask my mom if she wanted a shower it her shower day not a option short staffed often trying to get her transfer they giving me run around mom was taken off med three days later they still giving her the med she had food allergy n they gave her the food three time omg n few nice nursesn aide
